Pachyonychia Congenita Charity Golf Tournament

Held each August beginning in 2011, the tournament has attracted more sponsors and golfers each year. This year, the 6th Annual Pachyonychia Congenita Charity Golf Tournament will be held Saturday, August 13, 2016 at the beautiful Crater Springs Golf Course at the Homestead Resort in Midway, Utah.

GOLF PAYMENT INFORMATION

You can submit your payment by credit card online at GiveDirect (please note golf), calling PC Project at 801-401-6300, or by mailing a check made payable to PC Project to 2386 East Heritage Way, Suite B, P.O. Box 9010, Salt Lake City, UT 84109-0100

All proceeds are used for research and patient support services. If you register and pay before 1 May 2016, you can take a $25 discount off your total amount due.

  • Foursome $425 (for $375 more you can be a hole sponsor)
  • Single Player $125

SPONSORS (each of these includes a foursome golf team)

  • Presenting Sponsor – $1,000 + prize
  • Breakfast Sponsor – $1,500
  • Beverage Cart Sponsor $1,500
  • Hole Sponsor $800

INDIVIDUAL OR CORPORATE DONORS (no golf).

  • Tee Box Sign & advertisement in program $500
